SimplyIcon is freeware app that provide you make your own customize icons within no time. you can make icons in different sizes 128 x 128, 32 x 32, 24 x 24, and 16 x 16.
Note: SimplyIcon requires the .NET Framework (version 2.0 or higher).

SimplyIcon is simple app , there is no need for installation. All that you need to do is unzip the two files (SimplyIcon.exe & FreeImage.dll), place them in a folder and create the shortcut of the program where you want.
You just need to drag and drop the image in SimplyIcon's window area. It will also generate the 128x128 level if your source image is equal to or larger than 128x128.
